SEVERE IUGR and development...

summer1971 - 08:39pm Aug 30, 2007 EST

Hi.. I'll try not to make this too long winded!

My baby was delivered at 33 weeks weighing 630g (just under 1lb 6oz). Although he is an identical twin they did not have twin to twin transfusion syndrome. They had a placenta each (the egg split within 3 days of fertilisation) so it was just a case of placental insufficiency. We were warned when I was 19 weeks pregnant that Tom wouldn't make it much further into the pregnancy and we were also urged to consider a selective termination.
Somehow we made it to 33 weeks and Tom astounded everyone by being pretty much absolutely fine.... just very tiny. He was breathing completely on his own within 24 hours, off all his monitors within 2 weeks, and then spent 90 days in SCBU growing (feeding slightly hindered by his tongue tie, which I finally persuaded them to cut and he was off his NG tube and breast and bottle feeding normally and home within a week). He came home weighing 5lb 5oz at 3 months old.
I then continued to mix feed him. I had originally wanted to just breastfeed but it just wasn't happening so at each feed he had breast and then Nutriprem 2 (prescription only high cal formula ) top ups. His twin was breast fed only.
I'm now kicking myself for insisting on the breast for him for so long as I think his growth would have been far better had he been purely on the formula, but nobody suggested this to me at the time.
Anyway!... at 7 months corrected (will be 9 months actual next week) he is now 12lb 6oz to his twin's 17lb3oz which I guess doesn't sound too bad but hes still pottering along way below the 0.4th centile on the growth chart. Hes really dropped his milk intake since I started weaning him (only on Nutriprem 2 now as I dropped the breast nearly 2 months ago) so I'm grating loads of cheese into all his food and giving him Petits Filous fromage frais at every meal. Can't see when he'll ever catch up as his growth is steady and following the centile lines.

Right... my question is... how have your IUGR babies developed?... Tom's twin Jake was rolling over and over at 4 months (corrected) and sitting really well at 6 months and crawling from 5.5 months. Its sooo hard not to compare them and I know Jake is pretty advanced with his gross motor skills as my daughter didn't crawl 'til she was 10 months, but Tom is nowhere near even sitting up.
He can roll from his tummy to his back, which is actually all he does on his tummy. The second I put him down on his tum he flips himself over. When on his back he rolls from side to side.
We've got our next appt with his consultant on 10th Sept and hes seeing the physio then too.
Its just that I've started really worrying recently that maybe we're not going to get through this as unscathed as we have so far. If you know what I mean

Proud mommy of Emily Madeleine and Sydney Ann

Your boys are so adorable. My daugter also had severe IUGR- 28 weeker, weighing 15.5 ounces. She is now 19 months old and weighing about 15 pounds. She has come a long way and it is predicted she will be on the charts around 36 months. Her doctor is not concerned with her weight since her growth curve is steady. At your next appointment if you are concerned about weight, ask your doctor to look at the growth chart. If he is maintaining his growth curve then there is nothing to worry about.

As far as development goes, all kids are different. Even twins! The earlier you work with early intervention the better.. .Emily developed pretty close to her actual age and has now caught up to her real age in just about everything.

I hope all goes well!

Lori
