Need pictures for Signature Chefs Video

Hello everyone,
I am working on the Toledo, OH Signature Chefs Auction and we are in desperate need of photos for our mission video showing MOD families. We only have 3 pictures so far and obviously need much more so I am turning to other MOD families for help.

If you are interested in participating, please contact me.
1) High resolution picture of your premature child/child born with birth defect(s) from when the child was born and a current picture
2) High resolution picture of your healthy child both from birth and a current picture
3) Date of birth (if child was born prematurely or with a birth defect, # of weeks at birth or birth defect)
4) Length/weight of child at birth
5) Hometown
6) Any information you would like to share about a child that you lost
7) Child’s first name and parents first and last name (no last names will be used in the video)
Thank you for allowing us to utilize pictures of your child to spread the message, “Every baby needs to be born healthy.”
