2 failed Vaginal Cerclages, what next ??? Need ur success stories and strength

hope4me - 12:47pm Sep 23, 2009 EST

Hello Ladies,

I am new to this website. I am 32 and have had 3 consecutive miscarriages!!! I really need you women who have been through the same situation to give me some hope. I want to hear about your combat if u were in a situation similar 2 mine.

I have always been overweight(Pre Preg Wt 208 lbs). I took metformin as I had irregular menstrual periods and was diagnosed with Poly Cystic Ovaries (PCOD) when we were TTC. I have one kidney by birth (left one) and have 2 cervices with a septum. So, I have a Didelphyus Uterus with the left horn slightly larger than the left. The perinatologist feels that with this kind of a uterus shape I can conceive and give birth.

I conceived the first time through an assisted IUI cycle (Injections) and conceived twins in the right horn (smaller). Unfortunately, at 15 weeks I started cramping and lost my babies (boy and a girl). I did not have a D&C as everthing just made its way out. The doctors said that with barely half the uterus and a multiple gestation, this was bound to happen. I was not ready to buy this theory as dont women carry multiples ??? But this was my first pregnancy and so I juss did not think too much.

Within a month I conceived again with a singleton, this time naturally. It was in the left horn (larger). We thought that it would go well this time. But I needed an emergency cerclage at
17 weeks as I had dilated around 2 cm. My cervix was stiched. The pregnancy stayed for just 2 weeks more. I had a uterine infection (infection of the amniotic fluid). My baby (boy) had passed away and I had to be induced labor at 19.5 weeks. I needed a D&C this time. The doctors said that I had real bad luck with this one. Again, blaming it on bad luck did not appeal to me, but.....

After a year I conceived again in April this year. This time we wanted to ensure that there was to be a singleton in the larger horn. The doctors had also planned to put the stich at 12 weeks well before time to enhance my chances of a near to term pregnancy. I conceived again with a natural IUI (no meds) cycle. At 12.5 weeks, I had a cervical stitch put in place. From week 16, I started 17P Progestron shots(to relax the uterine muscles and prevent preterm labor). At 18 weeks, I started to have shooting lower back pains and when I went to the hospital, I was already 3-4 cm dilated. The stitch had to be removed and the water bag had to be broken to allow delivery. I lost my baby girl!!!

I am totally devasted. I have an abnormally shaped uterus. But dont women carry multiples ? I atleast have more than half of a normal uterine cavity ? Then why should I not carry a baby at least close to term ?? I had 2 consecutive cerclages but none of them worked. I have heard about a Trans Abdominal Cerclage(TAC) which works when a vaginal fails. But the TAC is a major procedure and I am scared that this should not affect my ability to get pregnant as it can be done before even TTC. What other are my options ? Can anyone suggest a very good Doctor in NYC who specializes in high-high risk cases ? Will I ever have a healthy living baby ?

Please tell me me how u dealt with an incompetent cervix or recurrent miscarriages. As much as I want to get pregnant, I am scared to death that I will not go beyond the 4th month.

I am sorry that you have been through so much. I have not been through IC personally but I know many women on here who have and I know they will be here to give you information. The only thing I can offer is that sometimes a second opinion helps. You may want to consult with another peri before trying to conceive again and they may have some different ideas.

Good luck and keep us posted.
Ellen

Thanks for ur support Ellen, I am trying to get hold of a high risk peri at Mt.Sinai Hospital in NYC for a second opinion.
On my follow up visit with my regular OB GYN, he suggested tht we test my blood for Thrombophilia (and some fancy blood tests) and also do a Urine culture to see if there are any antibodies which hinder the growing featus. My doc also mentioned considering a TAC as I have had 2 failed vaginal cerclages.

I just joined this site to see if anyone has had successful outcomes with a TAC. I really cannot imagin losing another baby!!!

hi

I am sorry about your losses, I recently had a TAC placed, and I wanted to share my experience with you, there are many doctors that do this and they may use different methods or do it before or after pregnancy. There is a website called abbyloopers, You need to go there and post your question there. It is a wonderful support system for all ladies with similar problems, and there is a list of the doctors there. People even fly from other countries to get this done by an expert. I did the same thing and I am very happy with my decision. If you have IC, you should get a TAC, and I will only get it by someone who is an expert and has done lots of this. email me if I can help you in any ways.

Good luck

Sheri

Mom to Kimberly (25 wkr, 6 yrs!) & Matthew (38.5 wkr, 4 yrs!)

I'm so sorry this has happened to you. It's just not fair. I know there are other women on this website with a bicornate uterus and that they have carried a baby to near term from what I recall. But having an bicornate uterus and IC - I'm not sure how that would work.

Women carry multiples all the time, but I don.t know how often women with two parts to their uterus do it. Since each side is smaller than normal, it's possible that it might not stretch enough to carry a multiple birth.

I would definitely look into the website Sheri mentioned - sounds like a fabulous place for information!

Hi Sheri

Thanks a bunch for replying. I am guessing that the TAC did wonders for you
I will surely post my query on that site.

It appears that since am so prone to infections, I should just get it done before getting pregnant. Will putting in a TAC affect my ability to get pregnant ? is it advisable to do the TAC after achieving pregnancy at 9-12 weeks ? I am in a dilema and really dont want history to repeat. I just re-collect being pregnant and loosing it for the last 2 years!! Sorry, dont mean to crib here, just have so many questions!!!

My OB GYN has prescribed some fancy blodd tests and 24 hr urine analysis test to rule out other hormonal imbalance tht hinder the pregnancy...

I will visit the site...Thanks Jackie as well..it means much tht someone is hearing me out

hi

Hi hope4me

I just had my TAC placed 2 weeks ago, I haven't tried it yet!! But I am glad I got it done. I feel that I may have close to a normal pregnancy now.
My understanding is that if you don't have problems with fertility, this doesn't change anything. There is also a IC and IVF forum too, ladies with both problems.
I talked to two surgeons on the phone, I got the names from the abbyloopers site, and decided on one, they were both great doctors. I was pregnant first, and was suppose to get it done at 10 weeks, but I lost the pregnancy early on, so I got it done pre- pregnancy.
I will ask these questions from the surgeons. In fact make a list of your questions. call or email for consultation, and you get all your questions answered.
You can do TAC after or before, I totally understand that you have a lot of questions, I did too. I just think about everything, what if this or that. But once you go to the abbylooper site, there is a section that they have asked 10 or 15 questions about TAC from few different doctors, and their replies are there. So that was really good.
My email is
Sherimom29@yahoo.com
I just joined here, and I might not check in often, so send me an email if you like.
I love to help you and I hope for no one to go through any loss like we have been through.
I had the blood tests done too I think, I know they recommended it, I just don't remember if I had it done now, but it doesn't hurt to rule all of those out.

Well, I am sorry to hear about your losses. I have had two miscarriages, but by grace alone I have four children. I had to lose them before they were able to tell me that my cervix was incompetent. I have had four cerclages and although I had complications with all of them, they worked out. I have dialated with the cerclage in and been put on bedrest. I have had the cerclage tighten and had to have it cut early so I could have the baby. It has a lot of complications that come with it but it is worth it, I suppose. All I can tell you is to have a little faith. I am supposed to have another cerclage soon and hope that everything works out this time too. I'm not supposed to be having kids, but I know that if God blessed me with four he will bless you in the same way. I hope everything works out.
